User Add: Openskedge Crashes with no logging

When adding a user and assigning a supervisor, Openskedge does not return information back to the client, and does not log the error.

Would like to also add that the apache error log states: child pid 22282 exit signal Segmentation fault

Also this is not only when adding a user but whenever you try to assign a user to a supervisor
