      <p>The source uses semicolons, repeats the <code>Name</code> header, leaves one header empty, and contains a duplicate data row.</p>
      <h2>Output</h2>
      <p>The normalized file renames only the ambiguous headers. The issue report identifies all three problems with row numbers.</p>
      <p class="boundary"><strong>No row is silently deleted.</strong> The duplicate remains in the normalized file because deleting a record can change business meaning. A paid cleanup removes or merges rows only when that rule is agreed in advance.</p>
