The review procedure document does not prompt leaders to do phase.estimation at the appropriate time clearly enough

The review procedure document describes 11 phases in section 4, one of which is phase.estimation. All phases except phase.request and phase.estimation have an associated procedure in section 5. These two phases should have their own procedure in the appropriate place in section 5 to comply better with rule.generic.clear and rule.generic.self

This was noticed when @thejayps discovered at or before proc.review.exit for #213 #223 and #230 that phase.estimation had not taken place.
