Add submission blacklist

Adds admin blacklists that stop users and servers from submitting to the gallery, and users from the public integration review queue. Also limits who can remove blacklist entries and manage integrations.

Blacklists

• New Admin pages:
  • Gallery Blacklist: users and servers; admin and owner.
  • Integration Blacklist: users only; owner only.
• Gallery: a listed user, or anyone submitting from a listed server, gets a 403 on submit, submit-tag, submit-form and resubmit.
• Integrations: a listed user gets a 403 when requesting public review, or when editing an integration that's public or pending review. Create, activate and delete are unaffected.

Permissions (narrowed for admins)

• Only the owner can remove entries, on all four blacklists (Gallery, Integration, Server, Global). Admins can still add to the Gallery and Server blacklists.
• Admin Integrations (list, detail, approve, reject, unapprove) and the "Integration Requests" notification are now owner-only.

UI

• Blacklist cards on all blacklist pages show an avatar or icon, a name and the ID, with fallbacks: default avatar, server-icon tile, "Unknown User"/"Unknown Server". The Server Blacklist can now be searched by server name.
• The grid is now 2 columns from lg and 3 from xl. Admin tabs grow to fit longer labels.
• Server names come from a new batch cache.GetGuilds. If the cache fails, the Server Blacklist returns entries without names instead of a 500, because AdminLayout's access check calls that route. The gallery and integration lists still return 500.
• Fix: faTrash and faSync weren't registered. So the trash icons (blacklists, SKUs, Polar products) and the Servers refresh icon never rendered, and the blacklist remove buttons, which are icon-only, were empty.
