Surface unresolved file authority claims for reconciliation

Summary

• Extend the read-only file authority inspector with a reconciliation list for spent claims that lack a local outcome or record an unknown callback outcome.
• Show only opaque grant IDs and operation/idempotency hashes; flag missing or mismatched claim, grant, operation, and outcome links.
• Treat unresolved local claims as needing review even when the authority lock is absent and checkpoint commitments agree. No lock removal, provider call, retry, or authority write is performed.

The report is a non-atomic local observation. An absent outcome does not prove the provider was called; a returned callback does not prove delivery.
