Ships the unified CLI/MCP operation surface from #38 and the issue ignore/unignore verbs from #40. Minor rather than patch: the MCP arg shapes changed (per-entity fields, plural list entities) and `list snippet-locations` is now paginated, so saved MCP call examples break; see CHANGELOG.md for the migration notes.
