Fastify 4 app bumped to v5

Medium Severity

Dependabot moved fastify from 4.x to 5.x in the node-fastify-4 E2E app, so this suite no longer exercises Fastify 4. The integration still documents support for Fastify v3.21.0–v6, and Fastify 5 is already covered by node-fastify-5.
