🔴 SQLite connections lack a close path
When a host creates stores repeatedly, createSqliteEventLogStore leaves every DatabaseSync connection open. The returned store exposes no close operation, so long-lived hosts can exhaust database handles.
Learn more
The factory opens a SQLite connection and returns only the four-method AgentEventLogStore interface. That interface has no method to release the connection. A host that constructs a store per session or request cannot close the connection after use. Connections remain open until the database object is collected, so repeated construction can exhaust file handles or retain database locks.
Example: A server creates one store for each of 1,000 requests to the same SQLite file. After each request finishes, none of the returned stores provides a way to close its database connection.
Recommended fix: Return an extended store with a close() or disposal method, and document that callers must close it. Alternatively accept a host-owned DatabaseSync instead of creating it inside the factory; ensure the test suite closes every store it constructs.

🟡 Empty Postgres appends accept stale indices
When entries is empty, append skips its only length check and accepts a stale expectedIndex. The store reports success where the store contract requires a conflict.
Learn more
The Postgres recipe implements an append-only thread store. Its reference append checks the thread length even for an empty entries array. Here, length is checked only by the conditional INSERT in the loop. An empty array executes no inserts, so a stale expectedIndex commits successfully instead of raising AgentEventLogConflictError.
Example: Thread t contains two entries. append({ threadId: 't', expectedIndex: 0, entries: [] }) resolves without error; the reference store rejects with actualIndex: 2.
Recommended fix: Check the current thread length inside the transaction before iterating, and raise AgentEventLogConflictError for any mismatch. Keep the conditional inserts for concurrent-writer detection.

🟡 Failed branch returns a successful draft
If the branch's draft request fails, runTimeTravelExample accepts its final failed state because b.status is "done". Callers receive a fabricated draft instead of a failure.
Learn more
The failed state is final, so reaching it produces a completed run with status done. The function checks only the run status, not which final state was reached. A failure in the branch's draft request therefore returns an output containing "The writer request failed.", and a host consuming b.output cannot distinguish it from a successful draft through the function's success path.
Example: An injected generateText succeeds for the outline and thread A draft, then throws for thread B draft. Thread B enters failed, but runTimeTravelExample returns normally with that error sentence as b.output.draft.
Recommended fix: After each run, inspect the snapshot for the failed final state and reject or expose the failure distinctly; keep the output success path for runs that reach done.

Check the thread length when entries is empty.
If entries is empty, this loop does not run. A stale expectedIndex then succeeds instead of raising AgentEventLogConflictError, contrary to the contract above. Check the length once inside the transaction before the loop.

Bind the actors to the per-run store.
The recallMemories and saveMemories actors above close over the single store binding. Passing those same actor instances to createAgentRuntime reuses that store; it does not swap stores per run. Construct replacement actors bound to the per-run store before passing them as overrides.

🌐 Web query:
official Node.js 22 node:sqlite DatabaseSync timeout default busy timeout
💡 Result:
In **Node.js 22**, `DatabaseSync`’s busy timeout defaults to **0 ms**—SQLite does not wait for a lock to clear before returning an error.
The `timeout` option was added in **Node.js 22.16.0**; for example, `new DatabaseSync(path, { timeout: 1000 })` sets it to 1,000 ms. ([nodejs.org](https://nodejs.org/download/release/latest-jod/docs/api/sqlite.html?utm_source=openai))
So, on versions **22.5.0–22.15.x**, there is no constructor `timeout` option; use `PRAGMA busy_timeout` instead. ([github.com](https://github.com/nodejs/nodejs.org/blob/main/apps/site/pages/en/blog/release/v22.5.0.md?utm_source=openai))

Configure a bounded SQLite busy timeout.
When another process holds the write lock, BEGIN IMMEDIATE can return SQLITE_BUSY before the store checks expectedIndex. This bypasses the conflict behavior required for competing appends. Use a bounded, configurable timeout for ordinary transient contention.
